Reporting
Designing a safety concern form crews will actually complete on construction sites
For subcontractors and safety managers: short, clear reporting forms get more reports. How to choose fields, order questions and keep a safety concern form quick to finish in the field.

Every field has a cost
For your subcontractor crews, use this guidance with the current work phase, trade responsibilities and approved procedures in view. Keep your safety manager involved in record reviews and follow-up. This guidance supports record keeping and coordination, not technical authorization to carry out the work.
Each extra question makes reporting slower. In the field, a long form means fewer reports, and the ones you get are rushed.
Keep the essentials
Ask for what the reviewer needs to triage.
- What did you see
- Where is it
- A photo
- Is it controlled right now
Leave detail to triage
Classification, root cause and severity scoring can be added by the reviewer. Reporters should only describe what they saw.
Test it on site
Ask a few crew members to complete the form on a phone, wearing gloves if that is normal. Fix anything that slows them down.
The takeaway
Ask only what triage needs, let reviewers add the rest, and test the form where it will be used.
